Sending out TTM requests isn't very hard, but it is important to do it right. If you miss a step, the player will be unable to sign your item.
1. Choose an item
Really anything is good. I have seen people send cards, pictures, baseballs, footballs, pucks, jerseys, batting gloves, bats, hats, index cards, custom cards, and postcards. If you can sign it, you can send it.
Keep in mind that the smaller and the flatter the object, the easier it is to ship.
2. Write LoR
You can't just send cards, otherwise the player will assume they are to keep. Write a letter to the player explaining what you want and thanking him. Here is an example:
Dear Mr. Jeter:
I am a fan of the Yankees and am intrigued by your career. I enjoy watching you play. Could you please sign this/these _____ for me? Thank you and good luck!
Make sure you are honest (you don't have 100 favorite players!), keep it short, ask for an auto, thank him, and wish him good luck.
3. Address envelope
This should be self explanatory, but you need to address the envelope.
4. Send SASE
Send a self addressed stamped envelope for the player to send the cards back in. If sending internationally, you will need postage from the country you expect they will be sending it from.
